These Terms of Service apply to Premier Chiropractic’s Spring Hill and Columbia websites, associated landing pages, online forms, scheduling tools, communication systems, and related online services.
1. Acceptance of Terms
By accessing or using a Premier Chiropractic website, submitting a form, requesting information, scheduling or requesting an appointment, subscribing to communications, interacting with an advertisement, or otherwise using our online services, you agree to these Terms of Service.
If you do not agree to these Terms, do not use our websites or online services.
These Terms govern website use and online interactions. Separate patient documents may apply when you seek or receive care, including:

Website content is not a substitute for an individualized examination, professional diagnosis, treatment recommendation, or consultation with a qualified healthcare provider.

3. No Medical Emergency Use
Premier Chiropractic websites, forms, emails, and text-message systems are not intended for emergency communication.
If you believe you are experiencing a medical emergency:

4. No Provider-Patient Relationship Through Website Use

A provider-patient relationship is established only after Premier Chiropractic has accepted you as a patient and completed any appropriate consultation, examination, evaluation, consent, and clinical documentation.
Submitting an appointment request does not require Premier Chiropractic to accept you as a patient or provide a particular service.
5. Chiropractic Services and Results
Premier Chiropractic may provide chiropractic examinations, chiropractic adjustments, neurological or musculoskeletal evaluations, digital X-rays when clinically appropriate, family chiropractic care, pediatric care, pregnancy-related chiropractic care, wellness services, and other services permitted by applicable law.
Not every service is appropriate for every individual.

- Symptoms;
- Pregnancy status;
- Medications;
- Previous injuries or treatments;
- Examination findings;
- Contraindications;
- Clinical appropriateness;
- Equipment availability;
- Insurance requirements; and
- Professional judgment.
Premier Chiropractic and its providers reserve the right to decline, postpone, modify, or discontinue a service when they determine that the service is not appropriate, clinically indicated, safe, lawful, or consistent with professional standards.
Individual Results Vary
Responses to chiropractic care and other services vary from person to person.
Premier Chiropractic does not guarantee:
- Pain relief;
- Symptom resolution;
- Improved mobility;
- Improved sleep or energy;
- A particular neurological or musculoskeletal outcome;
- Prevention of future symptoms;
- A particular number of visits;
- A specific treatment response;
- A complication-free result; or
- Any other specific clinical or wellness outcome.
Testimonials, reviews, success stories, service descriptions, educational content, and patient experiences describe individual experiences and do not guarantee that another patient will receive the same or similar results.
6. Appointments and Online Scheduling
Premier Chiropractic may provide online scheduling, appointment requests, telephone scheduling, app-based scheduling, third-party scheduling tools, or other booking options.
Submitting an appointment request does not guarantee:
- Appointment acceptance;
- A particular date or time;
- A particular provider;
- A particular service;
- Insurance coverage;
- Treatment eligibility; or
- A specific result.
An appointment is not final until confirmed through Premier Chiropractic’s applicable scheduling process.
Premier Chiropractic may reschedule, modify, or cancel appointments because of provider availability, staffing, emergencies, office closures, weather, equipment issues, clinical concerns, or other operational circumstances.
Certain services may require an examination, X-rays, screening, consent forms, financial arrangements, insurance verification, or provider authorization before care is provided.
7. Rescheduling, Cancellations, and Failed Appointments
Appointments are subject to Premier Chiropractic’s separate Active Patient Reschedule and Cancellation Policy and any other scheduling terms communicated to you.
Patients may be required to provide advance notice when canceling or rescheduling an appointment. Late cancellations, failed appointments, or no-shows may result in fees, adjustment of care-plan visits, calendar removal, account deactivation, or other consequences described in the applicable policy.
The separate cancellation policy available through the website footer is incorporated into these Terms by reference.
When a location-specific cancellation policy applies, the policy for the location at which the appointment is scheduled will control.
8. Pricing, Insurance, Payments, and Promotions
Prices, promotional offers, new-patient specials, service descriptions, and other financial information displayed on a website are provided for general convenience and may change without notice.
You are responsible for applicable charges associated with:
- Consultations;
- Examinations;
- X-rays;
- Chiropractic services;
- Care plans;
- Products, if offered;
- Missed appointments;
- Cancellation fees;
- Administrative charges;
- Insurance deductibles;
- Copayments;
- Coinsurance;
- Non-covered services; and
- Other agreed-upon amounts.
Insurance
Statements that Premier Chiropractic accepts or works with a particular insurer do not guarantee that:
- A particular service is covered;
- A provider is in network for your specific plan;
- Your insurer will authorize treatment;
- Your insurer will pay a claim;
- A quoted benefits estimate is accurate; or
- You will have no out-of-pocket responsibility.
Insurance verification is an estimate based on information available at the time of verification. The patient or responsible party remains responsible for charges not paid by an insurer, subject to applicable law and written financial agreements.
Promotions and Special Offers
Promotions may be subject to:
- New-patient requirements;
- Expiration dates;
- Service limitations;
- Location restrictions;
- Provider availability;
- Insurance-program restrictions;
- Medicare or other government-program restrictions;
- Clinical eligibility requirements; and
- Additional terms displayed with the offer.
Premier Chiropractic may modify, suspend, or discontinue a promotion when permitted by law.
Payments and Refunds
Payments may be processed by a third-party payment processor. Use of a third-party payment tool may be subject to that provider’s terms and privacy policy.
Refunds, credits, prepaid services, care plans, and payment arrangements are governed by the applicable written patient agreement or financial policy. Nothing displayed on the website creates a refund right beyond what is required by law or agreed to in writing.
9. Accurate Information and Patient Responsibilities
You agree to provide accurate, current, and complete information when submitting a form, requesting an appointment, completing intake documentation, seeking care, or communicating with Premier Chiropractic.
You agree to disclose relevant information concerning:
- Symptoms;
- Medical history;
- Previous injuries;
- Prior treatment;
- Medications;
- Allergies;
- Pregnancy;
- Surgical history;
- Existing diagnoses;
- Insurance coverage; and
- Other matters reasonably relevant to your care.
Failure to disclose material information may affect treatment eligibility, safety, insurance processing, or outcomes.

10. Communications and SMS/Text Messaging Terms
10.1 SMS Program
By affirmatively opting in to receive SMS messages from Premier Chiropractic, you agree to receive text messages related to your inquiry and your relationship with Premier Chiropractic.
Messages may include:
- Responses to questions;
- Information you requested;
- Appointment confirmations;
- Appointment reminders;
- Appointment-related follow-ups;
- Scheduling or rescheduling information;
- New-patient instructions;

- Billing-related communications;

Message frequency varies depending on your inquiry, appointments, communication preferences, and relationship with Premier Chiropractic.

10.2 Marketing Messages
Premier Chiropractic may send marketing or promotional text messages only when the applicable consent has been obtained.

You agree to notify Premier Chiropractic if you discontinue use of the number or the number is reassigned.
10.4 Opting Out
You may cancel the SMS service at any time by replying STOP to a Premier Chiropractic text message.
You may also reply STOP to (931) 225-9436.
After sending STOP, you may receive one confirmation message stating that you have been unsubscribed. You will no longer receive messages covered by that opt-out request unless you later provide new consent or re-subscribe through the appropriate process.
You may also communicate a clear request to stop messages through another reasonable method. Requests will be processed in accordance with applicable law and system capabilities.
10.5 Re-Subscribing
After opting out, you may re-subscribe by completing a new consent process or otherwise clearly requesting that messaging resume.

10.7 Carrier and Delivery Disclaimer
Wireless carriers are not liable for delayed or undelivered messages.

Questions concerning your text or data plan should be directed to your wireless provider.
10.8 SMS Privacy
Premier Chiropractic does not sell, rent, or share mobile telephone numbers, SMS consent records, or opt-in information with third parties or affiliates for their own marketing or promotional purposes.
Information may be provided to communications vendors and service providers only as reasonably necessary to deliver messages, operate the program, provide requested services, maintain records, prevent misuse, or comply with legal obligations.

11. Eligibility and Use by Minors
